Requirements

• A Valid CNA/HHA Certificate
• Minimum of 2 years' hospice experience required 
• Must have Alzheimer's and dementia training and experience 5+ years
• Reliable Transportation and Valid Driver's License
• Must be fully vaccinated w/ Booster and provide Covid Card 
• TB Test / Chest X-Ray within the last 12 months
• Must own and know how to use a smart phone
• Must be able to understand and speak English well

To be considered for this position, You MUST have a minimum of 2 + years of business development experience in  the Home care and or in the Hospice industry.  Your focus will be  hospital case managers, social workers, discharge planners, elder attorneys, geriatric care managers, executive directors of assisted living communities and skilled nursing facilities. We believe in connecting with the community, so being connected with local senior centers & community groups, is a PLUS!

RESPONSIBILITIES: 

·         Grow  the pipeline Of referral sources on a consistent basis

·         Measure KPIs and actively monitor performance accurately forecasting growth attainment

·         Log and maintain sales activity in CRM  

·         Work with leadership to create a go-to-market strategy for your exclusive territory 

·         Introduce Traditions Home Health Services to new contacts, including but not limited to, case managers, discharge planners, social workers, business development representatives and executive directors 

·         Identify & close new business opportunities within hospitals, rehab centers & assisted living facilities

·         Nurture existing customer relationships 

·         Provide competitive knowledge to help grow the business within existing channels

·         Attend networking events and groups that relate to your potential resources (virtual or in-person)

·         Present talks and educational seminars to B2B and B2C clients 

·         Daily communication with the scheduling team to foster synergy between sales and operations

·         Participate in advanced training programs

WHAT YOU BRING:

·        A minimum of 2+ years working in home care or skilled healthcare/Medicare sales.

·         Proven track record of meeting a quota  ( a minimum of 2 new private home care admissions each week )
